What is Transaxillary Breast Augmentation?

Transaxillary breast augmentation, also known as Scarless Breast Augmentation in San Jose, Los Gatos, and the surrounding Bay Area, improves breast shape by increasing breast volume with implants. The difference between a transaxillary underarm breast augmentation when compared to a traditional breast augmentation is the location of the incision.

In a transaxillary approach, a small incision in each underarm is made, through which the surgery is performed, using an endoscope (camera). Because it leaves only a small scar on the underarms that is hard to detect, it is called a “scarless” procedure. With other types of breast enhancement or augmentation procedures, scars are typically placed around the nipple or in the breast fold.

What are some of the advantages of Transaxillary Breast Augmentation?

  • No scarring on the breast tissue
  • Scars are almost undetectable
  • Optimal healing at the incision site

Endoscopic dissection allows for minimal blood loss and pinpoint accuracy in dissection leads to a quicker recovery and minimal discomfort after surgery

What are some of the disadvantages of Transaxillary Breast Augmentation?

  • Breast lifting cannot be done through a transaxillary approach
  • Less ability to fine-tune cleavage area
  • Less ability to shift muscle / dual plane to different degrees
  • Cannot combine with internal bra
  • Cannot combine with liposuction of the axillae

Transaxillary Breast Augmentation First Step

What’s the first step?

The first step in a transaxillary breast augmentation is to schedule a consultation with  Dr. Li at CA Aesthetics in Los Gatos. During this initial appointment, you will have the opportunity to discuss your goals, concerns, and medical history with Dr. Li. He will explain the procedure in detail, and help you understand your options regarding implant type, size, and placement. Dr. Li will work with you to develop a customized treatment plan tailored to your unique needs and desired outcomes.

If you and Dr. Li agree that this treatment is right for you, he will create your surgical plan. Once this plan is made, our Patient Coordinator will help you schedule your procedure date and provide you with a list of instructions to follow that will help you get the most out of your surgery and recovery.

What to expect on the day of Transaxillary Breast Augmentation surgery

Before you arrive at the surgery center, Dr. Li will give you pre-operative instructions to prepare you for surgery. Before being taken into the surgical room, Dr. Li will go through the surgical steps with you one more time to keep your mind at ease about the entire process.

Our Anesthesiologist will then administer your anesthesia to have you comfortably fall asleep and eliminate any chance of feeling pain or discomfort. Once you are fully sedated, Dr. Li will make the incision along the armpit, along the area of a natural skin fold. This way, the scar can be well hidden after healing.

Dr. Li then inserts an endoscope, which is a narrow fiber-optic camera through the incision site. This endoscope allows for viewing of the internal anatomical structures and guides him during the surgical procedure. The endoscope allows for viewing and accurate dissection of the pocket while minimizing bleeding. The implant pocket can be placed either behind or above the muscle of the chest wall. The implant pocket is the space in which the implant is positioned. The breast tissue and skin will stretch to accommodate the implant. The implant is then inserted through the underarm incision and into the created implant pocket. Dr. Li will evaluate the size, symmetry, and shape of the breasts and ensure that the implant is positioned properly.

Finally, once he is satisfied with the breast implant location and breast appearance according to the goals you discussed in your consultation, the incisions are closed with sutures, taking great care to carefully approximate the opening to result in minimal scarring. He will wrap your breasts in compression dressing to protect the recently operated tissues. You can also expect to wear a surgical support bra during the initial stages of your healing process.

The whole transaxillary breast augmentation surgery takes approximately two to three hours to complete.

Transaxillary Breast Augmentation recovery

After your surgery is complete, you will have time to awaken from the anesthesia in our recovery room where our team will monitor you. Before you leave to recover more comfortably at home, we will provide you with information about how to properly care for your breasts and follow-up instructions. Patients must rest for a few days to allow for full and proper healing of the tissues involved. Dr. Li will have you return the next day post-surgery so that he can check your healing and re-position any dressings if needed.

Exercise and heavy lifting should be avoided for four to six weeks, but patients may return to work and household tasks after one week. You will need to avoid shaving your armpits while you are in the initial healing period.
